Tidewater Emergency Medical Services Council, Inc. requests nominations for
the annual regional
EMS recognition
awards program.
About the Awards
The
purpose of the regional
EMS awards
program is to provide special recognition for outstanding contributions to
the local, regional or statewide
EMS system. Eleven categories, plus an additional high school
scholarship award, recognize
EMS field providers, EMS
instructors, EMS agencies, EMS agency administrators, public safety
communications officers, emergency nurses, operational medical directors,
contributions by those not affiliated with pre-hospital EMS services and
overall excellence in EMS.
Nominations
Any
person may submit a nomination for any award (s). The person making the
nomination should have knowledge of the nominee's qualifications for the
specific award category. A nominee's actions need not be
extraordinary…consistent and high quality work on a daily basis is often
deserving of special recognition. Many local EMS agencies provide
annual awards, and agency leaders should nominate those individuals for
regional awards. The nominee must have current licensure or certification in
the category in which he or she has been nominated. Nominations should be
made using the simple 2008 form or entering the information on-line
at
www.tidewaterems.org .
A brief resume and up to three letters of support, news articles or other
supporting documents may be attached to the form. Videotapes,
audiotapes and phone calls will be
considered by the awards committee. You may enter
and are encouraged to submit nominations at anytime through out the year.
However, Nominations must be received at the
Tidewater EMS
Council office no later than
Monday June 16, 2008
at 4:30 p.m.

Selection Process
A
selection committee consisting of last year's recipients and members of the
council’s EMS Promotions and Recognition Committee will select award
recipients. Members of the selection committee are not eligible to receive
an award. The Criteria and Eligibility information will be used to
evaluate nominations for each of the award categories. Persons submitting
nominations are urged to consider these criteria. The selection committee or
staff may correspond directly with nominees in order to obtain additional
eligibility and supporting information. The selection committee reviews the
nominations and supporting documentation and selects award recipients in all
categories. The selection committee's decisions are final. Presentation of
awards will be made during a special
EMS awards program in 2008
(Date to be announced). All
nominees will receive certificates of recognition; award recipients will
receive a special engraved award. Regional award recipients will be
forwarded as Virginia EMS award nominees in all categories except as noted
on the nomination form.
Governor’s
EMS Awards
Only regional winners
will be judged at the state level in the Governor’s EMS Awards program.
The State EMS
Advisory Board’s EMS Awards Selection Committee will review the first place
winners in each of the 11 Regional EMS Councils competition. Winners
selected at the state level will be forwarded with the Committee’s
recommendations to the Governor.
An EMS Advisory Board
Merit Award may be given to an exceptionally outstanding nominee who did not
meet all of the eligibility requirements for the Governor’s Award, but
deserved special recognition. If none of the applicants in a category meets
the awards criteria, the Committee can nominate someone. If the Committee
feels a nominee best fits another category, it may contact the nominator or
Regional EMS Council to discuss making this change.

Awards – Criteria and Eligibility
R.C.
DAILEY AWARD FOR EXCELLENCE IN EMERGENCY MEDICAL SERVICES
Criteria: For an
individual who exemplifies outstanding dedication and services to local,
regional or state EMS system and who has demonstrated a commitment to a
comprehensive, integrated system of EMS.
Eligibility:
Anyone
OUTSTANDING PRE-HOSPITAL PROVIDER
Criteria: An
individual who exemplifies outstanding dedication and service to their
community as a field provider.
Eligibility: Any
First Responder, EMT-Basic, Shock Trauma, Enhanced, Cardiac Technician,
Intermediate, or Paramedic with current
Virginia certification.
JAMES M. WAGENBACH OUTSTANDING EMS INSTRUCTOR
Criteria: An
emergency medical services instructor who exemplifies outstanding teaching
and leadership qualities and who demonstrates a dedication to the education
of prehospital providers.
Eligibility: Any
Virginia EMT Instructor and any other person who satisfies the above
criteria.
OUTSTANDING EMERGENCY MEDICAL SERVICES AGENCY
Criteria: For an
EMS agency which exemplifies outstanding professionalism and service to its
community; whose high level of patient care is evident by training programs,
community awareness programs, preventive health programs, and participation
in local, regional and statewide EMS systems.
Eligibility: Any
currently licensed EMS agency, affiliated auxiliary or support organization.
FRANK M. YEISER, JR. AWARD FOR OUTSTANDING OPERATIONAL MEDICAL DIRECTOR
Criteria: For an
operational medical director who exemplifies outstanding leadership and
dedication in the exercise of medical control in a local or regional EMS
system.
Eligibility: Any
licensed physician serving as an Operational Medical Director for a licensed
EMS agency.
OUTSTANDING EMERGENCY NURSE
Criteria: For an
emergency nurse who exemplifies outstanding dedication and service in the
field of nursing in the community in which he or she serves; demonstrates
knowledge and performance of EMS nursing skills; works well and
cooperatively with other
EMS personnel.
Eligibility: Any
Virginia emergency nurse (RN or LPN) currently serving in an active
position.
BRUCE W. EDWARDS AWARD FOR
OUTSTANDING EMS
ADMINISTRATOR
Criteria: For an
individual who exemplifies outstanding leadership and management qualities
in an administrative and/or field capacity. (Includes personnel at all
levels who have supervisory and/or administrative duties.)
Eligibility: Any
EMS person responsible for chairing, presiding over, supervising or
administering emergency medical services organizations and/or personnel.
OUTSTANDING CONTRIBUTION TO
EMS
Criteria: For
outstanding contribution to EMS by an individual, corporation, business,
etc. in providing financial, material and/or other tangible or non-tangible
assistance to support and benefit the local or regional EMS system.
Eligibility: Any
individual, corporation, business, etc. not actively and regularly
employed by, or volunteering with or directly affiliated with, the
prehospital EMS system.
OUTSTANDING EMS CALL -
Regional Only. No longer State recognized category.
Criteria: An
outstanding EMS call made between July 1, 2007 and June 1, 2008 that
demonstrates mutual aid, integration of multiple system components,
interagency interfacing and cooperation, or complex assessment or rescue.
The call must conform to standard EMS protocols of pre-hospital care,
including scene safety.
Eligibility: Any
EMS call within the region, from initiation into the system until resolution
at a health care facility, thus including the roles of the dispatcher, EMS
agencies, EMS providers, bystanders, hospital care providers and/or others
who provided EMS.
OUTSTANDING CONTRIBUTION TO
EMS FOR CHILDREN
Criteria: An
individual or organization that exemplifies outstanding dedication and
service to EMS for children.
Eligibility: Any
individual or organization with a significant contribution to the
development or enhancement of EMS for children in
Virginia.
Services cover a continuum of care, from injury prevention to pre-hospital
care, emergency department and hospital services, and rehabilitation.
OUTSTANDING PUBLIC SAFETY COMMUNICATIONS OFFICER
Criteria: For a
public safety telecommunicator (dispatcher) who exemplifies public trust,
looks out for the EMS personnel's safety and welfare and displays
professionalism in dealing with the public.
Eligibility: Any
individual primarily responsible for receiving information and assigning EMS
units to respond for help.
REGIONAL SCHOLARSHIP AWARD FOR OUTSTANDING CONTRIBUTION TO
EMS BY A HIGH SCHOOL SENIOR
This competitive $1000
scholarship will be awarded to a high school senior who has been affiliated
with an EMS agency within the Tidewater EMS Region for at least one year,
and who is currently enrolled or will be enrolled in an institution of
higher learning or an accredited ALS training program in the coming year.
Checklist
of items that must accompany this application form:

Proof of the
applicant’s affiliation with a Virginia licensed EMS agency. (Statement on
Agency Letterhead. Can be included as part of one of the letters of
recommendation)
□
Evidence of
high school senior status, with documentation of the applicant’s cumulative
grade point average. (No minimum GPA is required, but will be considered in
evaluations)
□
At least two
letters of recommendation on appropriate letterhead attesting that the
nominee’s academic and/or EMS performance is consistent with the highest
standards.
□
Digital

The individual
must reside and be affiliated with an EMS agency within the Tidewater EMS
region (Accomack, Chesapeake, Franklin, Isle of Wight, Norfolk, Northampton,
Portsmouth, Southampton, Suffolk or Virginia Beach).
